Follow the instructions below to use the form:
Step 1: Click on order form link & print form
Step 2: Look up your tax rate. Enter your mailing address and taxable amount; then click search/calculate tab. Scroll down to find your tax rate, sales tax and total due.
Step 3: Fill in the total due on the order form you printed and mail with check to:
